Services & PricingDOT SAP Evaluation Package — $550
For safety-sensitive employees who have violated DOT drug and alcohol regulations. Includes initial clinical assessment, treatment referral coordination, compliance monitoring, and follow-up evaluation in accordance with 49 CFR Part 40. Required before any prohibited employee can return to safety-sensitive duties.
Administrative Fitness-for-Duty Evaluation — $450
For employers requiring a clinical determination about an employee's ability to safely perform their role. Includes comprehensive evaluation, safety risk assessment, and return-to-work recommendations. Appropriate for non-DOT workplace behavioral concerns, post-incident assessments, and return-from-leave situations.

How It WorksIf you've never worked with a SAP evaluator before, here's what to expect when you contact me.
Step 1 — You reach out by phone or email. We briefly discuss the situation and confirm the evaluation is the right fit.
Step 2 — We schedule your in-person appointment. Same-week availability in most cases. Evening and weekend appointments available.
Step 3 — The evaluation is conducted at my Henderson, NC office. The process is thorough, confidential, and conducted in accordance with federal guidelines.
Step 4 — A written report is prepared with a clear clinical determination. No vague language. No ambiguity. Documentation that is defensible and ready for employer or regulatory review.
Step 5 — For DOT cases, I remain available for the required follow-up evaluation when the employee has completed their recommended program.

Frequently Asked Questions
Q: What triggers the need for a SAP evaluation?
A: A DOT SAP evaluation is required when a safety-sensitive employee tests positive on a drug test, tests 0.04 BAC or higher on an alcohol test, refuses to test, or adulterates or substitutes a specimen. The employee cannot return to safety-sensitive duties until the full SAP process is complete — regardless of how long they've been with the company.
Q: Does my small business fall under DOT regulations?
A: If your employees operate commercial motor vehicles over 26,000 pounds, you may be subject to DOT drug and alcohol testing requirements regardless of the size of your company. There is no small business exemption. If you're not sure whether your operation is covered, that's worth finding out before an audit tells you.
Q: What's the difference between treatment completion and SAP clearance?
A: They are two separate things. A program completion letter confirms the employee attended and finished their recommended program. Only the SAP can issue return-to-duty clearance — and that requires a separate follow-up evaluation. Employers who skip this step create a compliance gap even if the employee passed their drug test.
Q: Can my EAP handle the SAP evaluation?
A: No. If your EAP is already providing counseling to the employee, DOT regulations require a separate provider to conduct the evaluation. The conflict of interest disqualifies them. You need both — one provider for support and treatment, and a separate qualified SAP for the evaluation.
Q: How long does the process take?
A: The timeline depends entirely on how quickly the employee moves through each step — scheduling the evaluation, completing the recommended program, and returning for the follow-up. Employees who treat it like a job requirement and move promptly through each step return to work faster. I provide same-week initial appointments to remove delays on my end.
Q: Do you serve areas outside Henderson, NC?
A: Yes. I serve employers and employees within 50 miles of Henderson, including Oxford, Warrenton, Louisburg, Roxboro, Roanoke Rapids, and South Hill, VA. In-person evaluations only.
